Comments
When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.
An anonymized string created from your email address (also called a hash) may be provided to the Gravatar service to see if you are using it. The Gravatar service privacy policy is available here: https://automattic.com/privacy/. After approval of your comment, your profile picture is visible to the public in the context of your comment.
Media
If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.

Cookies
If you leave a comment on our site you may opt-in to saving your name, email address and website in cookies. These are for your convenience so that you do not have to fill in your details again when you leave another comment. These cookies will last for one year.

How long we retain your data
If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.
For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.
What rights you have over your data
If you have an account on this site, or have left comments, you can request to receive an exported file of the personal data we hold about you, including any data you have provided to us. You can also request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.

Your contact information
No contact information is held on this site, any contact information in relation to therapy received through the contact form is transferred to an enquiry form and kept for 7 years before being shredded in compliance with UK regulations and various professional associations of which I am a member (CNHC, AfSFH, NCIP, NCH).

This policy sets out the basis on which any personal data we collect from you, or that you provide to us, will be processed by us. Please read the following carefully to understand our views and practices regarding your personal data and how we will treat it.
Quest Hypnotherapy Ltd/Geraldine Joaquim will be a “controller” of the personal information that you provide to us, unless otherwise stated. Our data protection officer is Geraldine Joaquim. Our processing of data has never included the sharing of data with third parties with the exception of any venues used for the necessary delivery of a service. We have only acquired data through opt-in means. We use a password protected database system that is accessed only by our team.
We are not using sensitive data to determine profiling in any way.
When you register as a clinic client or opt in to our newsletter, we will ask you for personal information according to the service selected.
Clinic information collected
- Contact details – name, address, email address, telephone number, date of birth
- Family details/history, Doctor’s name and contact information
- Medical conditions and details of any regular medication that you take
- Various food, nutrition and lifestyle profile questions
In all case, taking your data is collected via a face-to-face consultation either in person or via Zoom, at the request of the client. In some cases where the client has come from a third party clinic there may be a situation where data sharing is beneficial to client outcomes, however at all times this would only be done under the advice of the client.
Newsletter
Our data for our newsletters ask for name and email from an opt-in form. This form could be online or a paper form filled out at an event, including an online event such as a webinar. All previous customers are automatically informed of future events and they are informed of this verbally and in writing with the ability to opt-out at any time.
Shop
If you buy from our online shop we only use this contact information to post you those goods, or send you a link to those goods if it is a digital good. We do not undertake any standalone postal marketing but we may include a leaflet with purchased goods.
Why we need your personal information
We need to collect our clients’ personal information so that we can manage your services. If you do not provide us with all of the personal information that we need to collect then this may affect our ability to offer the above services.

We may be required to share personal information with third party venue hosts.
How we protect your personal information
Your personal information is stored in hard copy only where possible, filed in a fire-proof locked cabinet and destroyed after 7 years. You may request access to your personal data at any time in writing by email or post, or verbally.
How long we keep your personal information
We will not delete personal information unless you ask us to. We never share your data with third parties other than venue or clinic partners and at all times our clients will be informed of this.
Your rights
You can exercise any of the following rights by writing to us at info@geraldinejoaquim.co.uk Your rights in relation to your personal information are: you have a right to request access to the personal information that we hold about you by making a “subject access request”;
- If you believe that any of your personal information is inaccurate or incomplete, you have a right to request that we correct or complete your personal information; you have a right to request that we restrict the processing of your personal information for specific purposes; and
- If you wish us to delete your personal information, you may request that we do so.
- Any requests received by Geraldine Joaquim/Quest Hypnotherapy Ltd will be considered under applicable data protection legislation. If you remain dissatisfied, you have a right to raise a complaint with the Information Commissioner’s Office at www.ico.org.uk

All online payments are via a secure server where all information across the Internet is being encrypted (we use Stripe). If you choose to use postal/shipping services an extra cost will be applied to your order. You can see this cost before accepting the order.
